70歳外国籍女性。元々84Kgあったが今は63Kg。減量はしたけれど、どうも腰回りから太ももの脚の具合がすっきりせず、皮膚の凸凹や血管が見えるのが気になる。以前に静脈瘤の手術を受けた事があり、このたび近医にて診てもらう予定とのこと。
脂肪浮腫はいわゆる外国籍女性に多い体型で、ウエストの割には腰回りが大きく、その流れで太ももも太いけれど、膝下はそうでもないことが多いです。さらに進むと膝下も太くなりむくんできます。
このような場合の対処法は基本的にまず減量が大切ですが、減量自体が困難な事が多く、また、この方のように減量しても腰回りの大きさや膝上がすっきりするにも時間がかかり、その意味でいったん脂肪がついてしまうとなかなか改善しにくいのが特徴でもあります。
それではどうするかと言うと、それでもやはり減量が最善です。この方のような皮膚の凹凸はおそらくセルライトー脂肪の塊ですので、次の段階はこれを改善するにはいわゆるリンパドレナージとなります。運動療法もリンパドレナージと同様の効果を期待できますので根気よく続けます。さらに、できれば弾性パンティストッキングを着用すると、圧迫効果とマッサージ効果が得られるのでより効果的です。蛇足ですが、その際の弾性パンティストッキングの選択はサイズ通り購入するときつい事が多いので気をつけます。
この方の”血管が見える”というのはおそらく静脈瘤で、大きなものでなければ見た目、すなわち美容上の問題はありますが、病的な問題はないことが多いです。
脂肪浮腫というのは日本人には大変少ないですが、ない訳ではありません。リンパ浮腫に加わって、リンパ浮腫を増悪させていることも多いです。時には脂肪浮腫のむくみをリンパ浮腫と診断され、様々な検査の後、リンパ管静脈吻合(LVA)などを施行されることもありますが、リンパ管の障害ではないので思ったような効果は期待できない事が多いと思われます。写真は脂肪浮腫で、LVAの効果が得られず当院を受診された方です。
リンパ浮腫は広く知られてきたのは喜ばしいのですが、リンパ浮腫でないむくみがリンパ浮腫として対応されてしまう状況が少々見られるのは注意したい点かと思われます。(廣田彰男)
A 70-year-old foreign woman. Her weight used to be 84 kg, but it is now 63 kg. Although she has lost weight, the area from her hips to her thighs still doesn’t look smooth, and she is bothered by the dimpled skin and visible veins.
Lipoedema is a body type rarely seen in Japanese women; while the hips are large relative to the waist, and the thighs are thick as a result, the area below the knees is often not as thick. As the condition progresses, the lower legs also become thicker and swollen.
In such cases, weight loss is fundamentally the most important first step. However, losing weight is often difficult in itself, and even when weight is lost—as in this patient’s case—it takes longer for the hip area and thighs to become slimmer. Therefore, once fat accumulates in these areas, it is notoriously difficult to improve.
So, what should be done? Even so, weight loss is still the best approach. The uneven skin texture seen in this patient is likely caused by cellulite—clumps of fat—so the next step to improve this is the so-called lymphatic drainage. Exercise therapy can also be expected to produce similar results to lymphatic drainage, so it’s important to continue it consistently. Furthermore, wearing compression pantyhose, if possible, is even more effective because it provides both compression and a massaging effect. Additionally, when selecting compression pantyhose, be careful—if you buy them exactly according to your size, they’re often too tight.
The fact that this patient’s “veins are visible” likely indicates varicose veins. Unless they’re large, while they may be a cosmetic concern, they often do not indicate a pathological problem.
Lipedema is very rare among Japanese people, but it does occur. It often coexists with lymphedema and can exacerbate it. Sometimes, swelling caused by lipedema is misdiagnosed as lymphedema, and after various tests, procedures such as lymphatic-venous anastomosis (LVA) may be performed. However, since this is not a lymphatic disorder, the expected results are often not achieved.
